Investigating the Integrity of Personal Radiation Protective Equipment (PRPE):

Personal radiation protective equipment (PRPE), such as lead aprons, plays a crucial role in minimizing radiation exposure for operators using X-ray systems. However, the integrity of PRPE can be compromised by cracks in the attenuating layer, potentially leading to inadequate radiation protection. This study aims to comprehensively examine the prevalence, qualification, and quantification of PRPE integrity over a longitudinal follow-up period.

Methodology
PRPE from a large, general hospital were evaluated annually between 2018 and 2021. The evaluation involved inspecting the equipment on a tele-operated X-ray table, with tears being assessed and measured using an X-ray opaque ruler. Lambert & McKeon rejection criteria were applied, with an additional criterion of 15 mm² for individual tears, to determine whether PRPE should continue to be used or rejected.

Results
Over the 4-year study period, a total of 1011 pieces of PRPE were evaluated. Alarmingly, 47.3% of the PRPE exhibited tears, with 31% of these tears exceeding the rejection criteria. Notably, among the 287 newly acquired pieces of PRPE, 6.0% displayed tears within the first year of use, and a significant 88.2% of these required rejection. Furthermore, 48% of repaired PRPE were rejected again in subsequent evaluations.

Conclusions
The study underscores the susceptibility of PRPE to developing cracks, with up to 50% showing tears and cracks, resulting in a substantial 31% rejection rate. Newly purchased PRPE do not guarantee freedom from tears in their initial year of use, and repairs do not ensure long-term integrity. Regular X-ray-based integrity assessments of PRPE are essential to ensure ongoing and adequate radioprotection for X-ray system operators.
